Long-term pain and health economic outcomes in adults receiving multidisciplinary CBT for chronic pain.

Introduction: The first-line evidence-based treatment for chronic pain is multidisciplinary treatment involving pain-focused CBT. Studies have shown that changes in psychological flexibility (PF) during pain-focused CBT positively mediatesoutcomes, even when the treatment does not specifically target PF. While CBT and PF-based treatments for chronic pain yield small to moderate, but clinically mea

Perceptions of important outcomes of moral case deliberations : a qualitative study among healthcare professionals in childhood cancer care

Background: In childhood cancer care, healthcare professionals must deal with several difficult moral situations in clinical practice. Previous studies show that morally difficult challenges are related to decisions on treatment limitations, infringing on the child's integrity and growing autonomy, and interprofessional conflicts. Research also shows that healthcare professionals have expressed a

Validation of the Viral Load Testing Criteria – an algorithm for targeted viral load testing in HIV-positive adults receiving antiretroviral therapy

Objectives: Restricted capacity for viral load (VL) testing is a major obstacle for antiretroviral therapy (ART) programmes in high-burden regions. Algorithms for targeted VL testing could help allocate laboratory resources rationally. We validated the performance of the Viral Load Testing Criteria (VLTC), an algorithm with satisfactory performance in derivation (sensitivity 91%, specificity 43%).

Case management for frail older people. Effects on healthcare utilisation, cost in relation to utility, and experiences of the intervention

Popular Abstract in Swedish Dagens vårdsystem står inför många stora utmaningar. Många av dem är relaterade till komplexa vårdbehov hos den allt större andelen sköra äldre personer. Denna andel ökar samtidigt som många vårdorganisationer har krav på sig att minska kostnaderna. Dessa äldre personer har ofta flera sjukdomar samtidigt och de har vanligtvis kontakter med många olika vårdgivare, på fleThe overall aim of this thesis was to investigate healthcare utilisation patterns and to explore the effects of a case management intervention for frail older people on healthcare utilisation and costs in relation to utility. A further aim was to explore the older people’s and case managers’ experiences of the intervention. Study I was a quantitative study comprising ten age cohorts aged between 6

Outcome in adult patients with severe TBI, using a management protocol allowing a cerebral perfusion pressure ≤ 60 mmHg – a retrospective study over 10 years

Background: Current guidelines for the treatment of severe TBI recommend maintaining a cerebral perfusion pressure (CPP) at 60–70 mmHg. In our institution, as well as others, an alternative algorithm—originally named the Lund concept—has been used. This treatment algorithm employs metoprolol and clonidine to limit CPP, accepting levels below 60 mmHg, with the aim of reducing cerebral edema. Previo

Inverse probability weighted generalised estimating equations for longitudinal data

Longitudinal study designs, in which variables of interest are observed at multiple time points in a study population, are frequently used in clinical research. Missing data are common in these types of studies. Moreover, in studies investigating a population where the mortality rate is high, data can be truncated by death. To handle missing or truncated data, it is important to investigate the un

Exploring Artificial Intelligence in Emergency Medicine for Predicting Disposition, Death, and Diagnosis

In the emergency department (ED), physicians and nurses make rapid, critical decisions. With the advancement of artificial intelligence (AI), we aimed to explore whether AI decision support could offer clinical benefits in the ED. Internet of things-linked wearable devices for managing food safety in the healthcare sector

Food, hygiene, and safety are critical in healthcare institutions, especially those in which patients are vulnerable against diseases triggered by food prepared under improper conditions. In the United Kingdom, almost 400 healthcare institutions were found to need significant improvements to their food preparation standards (ITV News, 2016). Such institutions still rely on traditional pen and pape

Visualizing healthcare system variability and resilience: a longitudinal study of patient movements following discharge from a Swedish psychiatric clinic

Background: As healthcare becomes increasingly complex, new methods are needed to identify weaknesses in the system that could lead to increased risk. Traditionally, the focus for patient safety is to study incident reports and adverse events, but that starting point has been contested with a new era of safety investigations: the analysis of everyday clinical work, and the resilient healthcare.
